Coverage Change — Birth or Adoption of Child
You may want to:
- Add your newborn child.
- Add your newly placed or adopted child.
Supporting documentation to submit
- A copy of the state or county certified birth certificate or adoption papers
- Universal Enrollment/Change Form (Health, Dental or Vision)
- Disability Enrollment/Change Form (Short-Term Disability or Long-Term Disability)
When to submit supporting documentation
Send to the Publix group benefits department by interoffice mail or fax to (863) 413-5771 within
- 60 days of the newborn's date of birth or
- 60 days of the child's date of adoption.
Other considerations
- FMLA: Discuss your eligibility for FMLA leave with your manager.
- Parental Leave Pay: With this benefit, you may be eligible to take time off with pay, consecutively or intermittently, within the 1st year after the birth or adoption of your child.
- Short-Term Disability Insurance: If you’re eligible and enrolled, this plan replaces a portion of your pay for up to 12 weeks due to maternity leave (giving birth).
- Pay deductions for benefits: If you'll miss multiple pay periods, consider sending in your health, dental, vision, short-term disability (part-time associates only) and long-term disability (full-time associates only) premium payments to Publix group benefits to continue your coverage. You'll receive a bill at your home mailing address. If you have a loan from the 401(k) SMART Plan, you may need to mail your payment to Voya.
